In signing this waiver, I am agreeing to follow all Tubac Pickleball Club (TPC) policies, procedures and Code of Ethics as directed by the TPC Bylaws and Board of Directors. I further understand that I have access to the Tubac Community Center building for water and use of the bathrooms only.
In consideration of Tubac Pickleball Club a 501(c)3 Non-Profit organization, or any of their sponsors, agents or representatives, I am voluntarily accepting this waiver and participation in this activity at the TPC courts and location. I, the undersigned, intending to be legally bound thereby, for myself, my heirs, executors and administrators, waive any and all rights and claims for damages I may have against TPC or any of its sponsors, agents or representatives, or any individual associated with TPC, their representatives, successors and assigns for any injuries, damages and liabilities suffered or incurred as a direct result of my participation in activities with TPC and at the Pickleball Courts located at the Santa Cruz County North Complex found at 50 North Bridge Road in Tubac Arizona.
I additionally permit the free use of my name, likeness and/or voice in newspapers, broadcasts, telecasts or TPC associated social media.
I further attest that I am physically fit to participate and compete in this event.
